You most likely need to change the color of the text that should be displayed. If text color and box color are white you'll just see white logically @__@:

Edit: Looking more into it the gender text is also not displayed and the buttons are moved to the left, which usually means there is no text at all. Maybe you're missing something. Who did this theme? I'd ask the person who did that for help maybe, if not I'm sure people here will try to help aswell.

It should say beside the boxes... if you use the default theme, you will notice it says it beside the boxes. In this theme, you seem to also be missing the word in the 'Submit' button.
